Continuing my previous post about Advantages and Weaknesses of Questionnaire, I will post about the Definition and Types of Questionnaires.
The definition of questionnaire is a number of written questions that are used to obtain information from respondents in terms of his personal statement, or the things that he knows.
Questionnaire can be used to refer to method and instrument. So, in using questionnaire method, the instrument used is questionnaire.
Types of questionnaires are:
1. Types of questionnaires in terms of how to answer are:
a. Open Questionnaire
It is a questionnaire which provides an opportunity for respondents to answer in their own words.
b. Close Questionnaire
It is a questionnaire that has been provided the answers and the respondents have to choose.
2. Types of questionnaires in terms of the answer given are:
a. Direct Questionnaire
It is s a questionnaire which the respondents answer about them.
b. Indirect Questionnaire
It is a questionnaire which the respondents answer about others.
3. Types of questionnaires in terms of the forms are:
a. Multiple Choice Questionnaire
Multiple-choice questionnaire is same with close questionnaire in which the respondents have to choose the answers available.
b. Filling Questionnaire
This questionnaire is same with open questionnaire in which the respondents are given the opportunity to respond with their own words.
c. Check List
It is a list, in which the respondents have to sign the check or tick (√) in the appropriate column.
d. Rating Scale
It is a statement followed by columns that show the levels, for example, very agree, agree, disagree, and very disagree.
